Canany <br />Paonia, CO 81428 Division Director <br />/ / ,/ Natural Resource Trustee <br />Re: North Fork Pit, File No. M-1004-025, Approve! of Trander of Appoved Mineral Application and Succession of <br />Permit Applicant, Request for Financial Warranty and Performance Warranty, Revision No. SO-I <br />Dear Ms. Farnsworth: <br />On February 16, 2005, the Division of Minerals and Geology approved the Transfer of Approved Ivhneral Applicaion and <br />Succession of Permit Applicant from Oldcastle SW Group, Inc. dba United Companies of Mesa County to Farnsworth Construction <br />& Gravel Co. for the North Fork Pit, File No. M-2004-025. Farnsworth Construction & Gravel Co. is now the approved <br />application holder of the North Fork Pit, and as such, will be responsible for all provisions in File No. M-2004-025, as well as <br />those specified in the Rules and Regulations when the Division receives and approves the performance and financial warranties. <br />Oldcastle SW Group, Inc. dba United Companies of Mesa County is relieved of all responsibilities concerning this operation. <br />As a reminder: <br />The amount of financial warranty set by the Mvision for this operation is $22,212.00. Please see the enclosed reclamation bond <br />calculation estimate. You must submit a financial warranty in this amount and a performance warranty in order for us to issue a <br />permit. In the event you have requested a financial warranty form, we have enclosed it in this letter. If you have not, please <br />select a type of financial warranty firm Rule 4.3. Then centact us so that we can provide you with the appropriate warranty form <br />or you may access the Division's Web page by going to www mining cfatr rn nc then go into the "Forms, Policies and <br />Quidelines" heading to find Mmeral Forms, Mineral Warranty Forms. All of the bond forms and the Performance Warranty <br />form can be located on our web site. <br />PLEASE NOTE THAT MINING OPERATIONS MAY NOT COMMENCE UNTIL A PERMIT HAS BEEN ISSUED BY <br />THE DIVISION AEJ:EB RECEIl'T OF XOUR FINANCIAL WARRANTY AND PERFORMANCE WARRANTY. A <br />PERMTT WIId.
