Gunnison No. 1 Mine, File No. C-001-80 <br />Dear Ms. Beville: <br />The Division has completed its review of your minor revision application <br />concerning the addition of 4 to 8 groundwater monitoring wells submitted <br />July 15, 1985. <br />We are approving the revision with the following stipulations: <br />1. One copy of the following must be submitted within 30 days after each <br />calendar year to the BLM Chief Branch of Solid Minerals: <br />A. Lithologic and any other logs of strata penetrated and conditions <br />encountered. <br />B. Drill hole location map showing exact hole locations, each hole <br />being appropriately identified by number or letter with the data <br />being tied to such number or letter designation. <br />C. List of hole numbers and depth drilled. <br />2. All ground water monitoring wells must be completed and cemented so as <br />to isolate all aquifer intervals which show significant head <br />differences or changes in water quality in order to prevent mixing of <br />unlike waters. <br />3. All wells must be completed and abandoned in accordance with applicable <br />]aws of the State Engineer's Office. <br />4. Upon final abandonment of the wells, all water bearing zones shall be <br />sealed with a cement plug beginning 20 feet below the zone and ending <br />20 feet above the zone, or the entire well can be sealed with cement <br />from the bottom to within 3 feet of the surface. <br />5. Dry holes or holes sealed in intervals shall be permanently plugged <br />with an expanding plug placed 10 feet below ground level followed by 7 <br />feet of cement. <br />6.
