The amendment application must include all information and exhibits which are to be modified through this <br />amendment. The maps will comprise the most important materials of the amendment application, since they will <br />change the most. If you do not plan to change the method of mining or the details of the reclamation plan, those <br />exhibits might not require submittal of much new material, if any at all. <br />I suggest you refer to the detailed description of the material to be included in the exhibits (those additional <br />documents you prepare to accompany the application packet). The details for Exhibits A through L are shown in <br />Construction Materials Rules 6.3.1 through 6.3.12. Also, please refer to Rule 6.2.1(2) for the map standards we <br />require. (The Rules and Regulations are the half-inch thick packet oftan-colored pages you received from me a <br />few months ago. The Rules are also available on our website: www.mining.state.co.us) As I mentioned above, <br />updated materials are required only for those portions of the permit which you want to change. For all other <br />portions, you must still include an exhibit for them, but you may reference the original file materials from 1978, <br />and state that the earlier file materials still apply. <br />Your submittal should be sent to Ms. Suzi Ericksen at our Denver office (see address on letterhead) who will <br />review it to ensure that all the necessary portions are included. This will require that you send out some notices to <br />certain offices or individuals, and place a copy at your county clerk's office. Once Ms. Ericksen has ensured that <br />the amendment is "complete" she will notify you to place a notice to be published in a paper in your area. <br />Finally, please be reminded that a 110 permit amendment carries a fee of $719, which must accompany the rest of <br />the submittal, or we cannot begin our review. <br />Throughout all steps of this process, please do not hesitate to contact either Ms. Ericksen at the Denver office, or <br />myself at the Durango field office (see below) if there aze any questions.
