On October 8, 2002, the Division of Minerals and Geology received your 112 Cons[rucfion Materials Reclamation Permit
Amendment Application package for the Hogue/River Pit, Permit No. M-1992-066. Preliminary review of the information
received determined that the following items must be received before the Division can consider your application as being
submitted and technical review can begin:
APPLICATION FORM: Page 1, Item 1: The Division requires [hat the Applicant/Operator name be the same as
the name on the permit (i.e Duckels Construction, Inc.). Page 1, Item 3: The Division requires the exact acreage.
Page 2, Item 9: Quarter Section and Quarter/Quarter Section need completing. Page 3, Correspondence
Information: The Division requires that the Applicant/Operator name be the same as the name on the permit. Page
7, Signature Page: The Division requires that the Applicant/Operatorttame be the same as the name on the permit.

6.4.17): Please submit proof of actual delivery or proof of mailing by Certified Mail, return receipt requested, of
Notice of the Application to [he local Boazd of County Cormrlissioners.
Your application will not be considered submitted until the information listed above is received and found sufficient to begin
our review. A decision date will be established 90 days from the date of receipt of all of [he requested information.
Additionally, if you have already published notice you will need to republish notice, after the Division considers the
application submitted. This notice must be published once a week for four (4) consecutive weeks, starting within [en (lOp
days of the date your application is considered submitted. We will notify you when you should initiate republication of your
notice. The final date for receiving~commen[s is the 20th day~after [he fourth-publication or the next~regulaz~business day. - - - -
This letter shall not be construed to mean that there are no other technical deficiencies in your application. The Division will
review your application to determine whether i[ is adequate to meet [he requirements of [he Act after submittal of all required
items.
