The cover sheet of the application indicates no increase in bond. <br />Rule 2.08.4(5)(d) Such other information n.s maybe necessary for the Division to determine ijthe <br />proposed revision will comply with the Act and these Rtrles. The Board or Division may reasonably <br />request additional information to evaluate the proposed revision. <br />The application is in compliance with this rule. Information in the application is complete. <br />Rule 2.08.4(6)(c) Minor revision. <br />Rule 2.08.4(6)(c)(i): Immediately upon receip[ of a complete application for a minor revision, <br />the Division shall post a notice of the application in [he off ce of the Division and make a copy of <br />the application available for the public to irtspec[ and copy at a reasonable cost. <br />A notice of the application was posted on the bulletin board of the Division on March 23, 1999. <br />Rule 2.08.4(6)(e)(ii): Within 10 days after tl:e filing of a complete application for a minor <br />revision, the Division shall issue a decision approving or denying the application in whole or in <br />part'. A written copy of the decision shall be promptly provided to the permittee' and shall be <br />posted in [he Division offce'. <br />~ A proposed decision was issued, as of March 23, 1999 (Signed March 24, 1999). <br />' A written copy of the proposed decision was provided to the permittee under a letter dated <br />March 24, 1999. <br />' A written copy of the proposed decision was posted in the Division office (bulletin board) on <br />March 24, 1999. <br />Rule 2.08.4(6)(iii) Within 10 days after the notification required by 2.08.4(5)(c)(ii) above, any <br />person which has an interest which is or may be adversely affected may request a formal hearing <br />on the decision by the Division. Such request shall be in writing and state with reasonable <br />specificity the reasons for the request and objections to the Division. If so requested the formal <br />hearing shall be conducted in accordance with the requirements of 2.07.4(3). <br />A written copy of the decision was posted in the Division office (bulletin board) from date to <br />
