The Division requests that Williams Fork Company submit <br />a signed notarized statement that Trapper Mining, Inc. has the legal authority <br />to submit amendments and revisions to Permit No. M-84-168 on behalf of <br />Williams Fork Company. <br />Failure to submit this statement may result in a decision to deny your <br />revision.
