Butala Pit, M-87-127 <br />Dear Mr. Butala: <br />The Division received your request to extend the corrective action due date to November 19, 1998 for <br />the following problems which were identified during the July 15, 1998 monitoring inspection: 1) no mine <br />ID sign, 2) permit boundaries not adequately delineated and 3) no activity at the mine for several years. <br />The required corrective actions were: 1) place a mine ID sign at the entrance of the site which shall be <br />clearly visible from the access road, 2) mark the permit boundaries with monuments or other markers <br />that are clearly visible and adequate to delineate such boundaries and 3) the operator must either subtnit a <br />request for temporary cessation (pursuant to Rule 1.13) or reactivate mining operations of begin <br />reclamation. The operator was required to notify the Division by September 21, 1998 which option he <br />will select. <br />There were two other problems noted during the inspection. First, several rills and gullies have <br />developed on the west side of the terrace outslope. Some sediments are being washed out away from the <br />toe of the outslope. The operator was required to submit a technical revision to propose a plan to repair <br />the erosional features and a proposal to prevent future problems. The T.R. was submitted to the Division <br />on August 27, 1998 and is currently being reviewed so this corrective action due date will not be <br />extended. Second, inaccurate annual reports are being submitted. The operator must accurately report <br />the status of the operation each year when submitting [he annual report. The corrective action due date is <br />future reports so this due date will not be extended. <br />The request to extend the corrective action due date, for the three problems noted in paragraph I above, <br />is hereby approved.
