Butala Pit, M-87-127
Dear Mr. Butala:
The Division received your request to extend the corrective action due date to November 19, 1998 for
the following problems which were identified during the July 15, 1998 monitoring inspection: 1) no mine
ID sign, 2) permit boundaries not adequately delineated and 3) no activity at the mine for several years.
The required corrective actions were: 1) place a mine ID sign at the entrance of the site which shall be
clearly visible from the access road, 2) mark the permit boundaries with monuments or other markers
that are clearly visible and adequate to delineate such boundaries and 3) the operator must either subtnit a
request for temporary cessation (pursuant to Rule 1.13) or reactivate mining operations of begin
reclamation. The operator was required to notify the Division by September 21, 1998 which option he
will select.
There were two other problems noted during the inspection. First, several rills and gullies have
developed on the west side of the terrace outslope. Some sediments are being washed out away from the
toe of the outslope. The operator was required to submit a technical revision to propose a plan to repair
the erosional features and a proposal to prevent future problems. The T.R. was submitted to the Division
on August 27, 1998 and is currently being reviewed so this corrective action due date will not be
extended. Second, inaccurate annual reports are being submitted. The operator must accurately report
the status of the operation each year when submitting [he annual report. The corrective action due date is
future reports so this due date will not be extended.
The request to extend the corrective action due date, for the three problems noted in paragraph I above,
is hereby approved. The new corrective action due date is November 19, 1998.
If you have any questions, please give me a call.
Sincerely, cc
t it.~q,~ r
i / ~~1
' ames Dillie
Environmental Protection Specialist
