25. There are no engbreer's cer[ificntiorts on the design drawings 2.05-MSC, "Sedirneruation Pond C ", 2.05-MSE, "East Yard Sedimentation Porrd ", 2.05-MSG, "Upper Hubbard Gulch Sedimentation Pond " cord 2.05-MSH, "Sanborn Creek Mine Dewntering Pond. " Please provide revised drawings that hove the certifications. OMLLC will be submitting a revised drawing of Map 2.05-MSC for Pond C. The revised drawing was requested to add the signature of the P.E. who was certifying the designs and to change the storage capacity table to state Pond C, instead of Pond B. In the November 2001 submittal, OMLLC supplied labels that are [o be attached to the original certified copies of the pond drawings for three ponds. The East Yard Pond as- built drawing is to be labeled as Map 2.05-MSE, the Upper Hubbard Gulch Pond as-built drawing is to be labeled as Map 2.05-MSG and the Sanbom Creek Mine De-watering Pond as-built drawing is to be labeled as Map 2.05-MSH. The uncertified drawings for these three ponds that were submitted in PR-04 will not be placed in the permit application. 26. /t appears drat there are no ns-built certifications for the sedimentation ponds drat have been built nt the Sanborn Creek Mine. An ns-built certification should state that the pond in question has beers built according to the designs. Please either stole where in dre penrrit application these certifications are or provide such certifications. The Division will add a stipulation to the approval of PR-04 that will require the future submittal of the sediment pond as-built certifications. 27. For a clearer picture of the way rite pond is constructed, the Division believes thn! several of the pond drawings should show the positions of the primary atd/or emergency spi[hvays in bout the cross sections and plan views, even t{rotrg{t the spillways might rrot fall along the cross sectional slice of the drawing. Drawing 2.05-MSA does not show, in either dre plan view or the cross section view, that a portion of the road aces as an emergency spit/way. Drawing 2.05-MSB does not show the primary spillway in either the plan view or the cross section view. On dre scone drawing, the emergency spillway is not shorwt on the cross section view. Drawing 2.05-MSG does not show the prirrmry and emergency pipes in dre cross section view. On drawing 2.05-MSH, the primary and emergency spillway pipes are not shown on the cross section view. Please provide revised drawings. The stipulation described in item number 26 above will include the requirement for the future submittal of the sediment pond spillway information.
