Please be advised that on October 12, 2004, L.G. Everist, Inc., whose address and telephone number aze 7321 E. 88th
Avenue, Suite 200, Henderson, CO 80640; [phone], filed an Amendment application to M-1999-120, Ft. Lupton
Sand and Gravel Mine, which is located at or near Section 30, Township 2N, Range 66W, 6th Prime Meridian, in Weld
Crnmty. The application decision is scheduled for January 10, 2005.
A wpy of the application is available for review, unless you receive a CD, at the Weld County Clerk & Recorder's
office and at the office of the Division of Minerals and Geology, Room 215, 1313 Shennan Street, Denver,

If you desire to make comments or objections of the mine plan and/or the reclamation plan, they must be submitted
within twenty (20) days of the date of the newspaper public notice to be considered in the application review process by
