This action was completed by receipt of such a revised page by facsimile <br />on November 11, 1999 (hard copy to follow by mail). <br />Item 2: A revision to revised (PR-3 submission) page 89, line 4, to add: "This pond is inspected <br />by a registered professional engineer, and reports submitted to the Division of Minerals and <br />Geology, quarterly.", or words to that effect. The purpose was to designate submission of the <br />inspection reports on a quarterly or annual basis [Rule 4.05.6(13)]. A revised page to that effect <br />was received on November 11, 1999, by facsimile (a hazd copy to follow by mail). <br />Item 3: Proof of publication of the public notice of filing the application for PR-3). Such proof <br />was received on November 11, 1999, by facsimile. <br />Item 4: A certification of insurance in the form required by the Rules (see Division letter of <br />October 25, 1999, Subject: Certificate of Liability Insurance). <br />Item 5.
