On March 1, 1994 the AVS was queried for the Edna permit revision
03, permit number C-80-001. The system recommendation was ISSUE.
I have a few AVS adequacy concerns as a result of this AVS check
that are mentioned below. Please have the operator submit a minor
revision to the permit application to update the officers and
directors of P&M.
1. It appears that the permit pages, outlining the identification
of interest section of the permit, are outdated. The pages I
used out of the permit date back to 1990 and 1991. The AVS
appears to have the most accurate information, so I based my
review from what was in the system. Below is a list of
officers and directors that were listed in AVS but not the
permit application pages.
