October 2004 -26- 043-2170 <br />• 5.0 GEOSYNTHETICS CQC SUBMITTALS <br />Geosynthetics CQC submittals reviewed by Golder included installation personnel resumes, <br />geomembrane resin certificates, geomembrane roll certificates, welding rod certificates, geogrid <br />certificates, and geotextile certificates. Golder's inventory control, summazizing CQC certification <br />received for each roll of geomembrane received for the Phase Nc composite liner system, is <br />presented in Appendix J-1. All geosynthetic material used as part of the Phase Nc OSA construction <br />met project specifications. <br />5.1 Geomembrane Installation Personnel Resumes <br />Golder reviewed the resumes of Poly-Flex installation personnel to verify that the installation <br />superintendent and the seaming personnel possessed sufficient experience as required by the project <br />specifications. Review of the installation personnel resumes indicated that key personnel had <br />experience sufficient to meet or exceed project specifications. Geomembrane personnel resumes are <br />presented in Appendix J-2. <br />• 5.2 Geomembrane CQC Documents <br />CQC documents submitted by Poly-Flex included resin supplier certificates and geomembrane <br />manufacturer resin certificates, geomembrane roll certificates, and welding rod certificates. Golder <br />reviewed CQC documents prior to geomembrane placement and verified that the documentation was <br />complete and that the material properties and CQC testing frequency required by specifications were <br />met. <br />5.2.1 Resin COC Certificates <br />LLAPE polymer raw material (resin) CQC certificates were provided by the resin manufacturer, <br />Chevron Phillips Chemical Company, at a frequency of one per rail caz shipment. Golder reviewed <br />these certificates prior to geomembrane placement. The geomembrane manufacturer also performed <br />density and melt index tests of the resin supplied by Chevron Phillips Chemical Company to verify <br />material properties prior to geomembrane manufacture.
