October 2004 -2- 043-2170 <br />• quantities and testing frequencies presented in this report aze current as of October 2004, which <br />represents completion of the Phase Nc OSA, as detailed in this report. <br />1.2 Construction Quality Assurance/Quality Control <br />Golder provided CQA and CQC monitoring and testing services, and prepazed this report describing <br />the monitoring and test results for the Phase Nc OSA activities. As-built drawings and supporting <br />CQA and CQC monitoring and testing documentation aze presented in appendices attached to this <br />report. <br />1.2.1 COA Monitoring of Earthworks <br />Golder provided CQC testing and CQA testing and monitoring of earthworks for the Phase Nc OSA. <br />CQA testing and monitoring of earthworks was provided for in-place materials in accordance with the <br />Golder Earthworks CQA Plan. The earthworks material specifications for this project are <br />summarized in Table 1. CQC earthworks testing was provided to support the contractor during the <br />manufacture of materials and prior to material placement. The CQC test results are not included in <br />• this report because these tests were intended to assist the contractor in materials prepazation rather <br />than record testing. <br />Golder provided CQA laboratory and field testing, and monitored earthworks construction activities <br />as follows: <br />• Golder visually monitored and documented the remediation of underground <br />workings within the Phase Nc OSA footprint, including verification that Ames <br />excavated underground workings to the minimum specified dimensions and then <br />installed Coazse Shaft Backfill (CSB), Concrete Plug (CP), Cemented Rockfill <br />(CR), Select Structural Fill (S5F), Structural Fill (SF), and geogrid as specified on <br />the Issued For Construction (IFC) drawings. <br />• Golder monitored construction of the Secondary Underdrain (SU), including <br />verification that Ames excavated trenches to minimum required dimensions and <br />grades, and placed materials as specified. Golder performed CQA laboratory,testing <br />of grain size, Atterberg limits, and moisture content of the Underdrain Fill (I3F) and <br />SSF placed in the Underdrains. <br />• Golder monitored and observed construction of the permanent storm water <br />diversion channel outside the perimeter of the Phase Nc OSA, which includes <br />• verification of properly excavated diversion channel dimensions, removal of <br />~:~.~~,~,any.~rv~.~«„o.,z„o.o.~~:,a~~.MS,.-..a« Golder Associates <br />
