Teller Sheet, <br />Lakewood, Colorado 80235, and: <br />Left Nand Wat[x District <br />F.O. Box 210 <br />Niwot, Colorado 80544-0210 <br />hereinafter relerred to as the "Structure Owner". <br />'Ihe Colorado Mined Land Reclamati«t Act requees that landowners adjacent to a proposed mining <br />operation be informed about the mine and the reclamation. Further, the Act provides protection for structures that <br />are within 20(1 feet of a mining operation. <br />Aggregate lndustries, has submitted an amendment application to the State of Colorado for its Division of <br />Minerals and Geobgy Regular 112 Operation P[rmit Nu. M-1989-029, and an am~dntrnt application to USR 877 <br />within Weld (;ounty, for sand and gravel mining located az or near Sections 7, 8, I7 and 14, T2N, R68W ofthe 6m <br />P,M., Weld Cottnq', Colorado. <br />The Slruchve Owner owns a permanent man-made structure or structures within 200 feet of the land <br />affected by the mining and reclamation operations. The description of the shuchve(s) is as follows: <br />The water Gne running north-south along Weld County Road E. <br />.Aggregate Industries must obtain permission 6om Structure Owner in order to conduct operations closer <br />than 200 feet to the structures owned by Svucture Owner. )n addition, Aggregate Industries must commit to <br />repairing, replacing or compensating Structure Owner for any damage Urat might be caused to its slrucure(s) by the <br />mining operation of Aggregate lndustries at this site. <br />By this agreement, Aggregate Industries requests authorisation from Structure Owner to conduct mining <br />operations up to a 50-fart setback from Structure Owner's structwe(s).
