Preliminary review of the information received <br />determined that the following items must be received before the Division can consider your application as being submitted and <br />technical review can begin: <br />APPT.iCATi(1N FDRMI page 1, Item 1: The Division requires that the Pemd[tee name be the same as the name on the <br />Pemdt. Page 3, Item 14: The Division requtres that the contact person be from [he same company as the Permi[tee or <br />someone authorized to submit items on the Pennittee's behalf. Page 5, Responsibilities as a Pemtittee: The Division <br />requires that these be initialed by a corporate officer of the curent Permittee. A decision date will be established 90 days from the date of receipt of all of the requested information. Additionally, if <br />you have already published notice you will need to republish notice, after the Division considers the application submitted. This <br />notice must be published once a week for four (4) consecutive weeks, starting within ten (10) days of the date your application is <br />considered submitted. We will notify you when you should ittitia[e repablicadon of your notice. - The fmal date for receiving <br />comments is [he 20th day after the fourth publication or [he next regulaz business day. <br />This letter shall not be constmed to mean that there are no other technical deficiencies in your application.
