release for the Joker Mine, Permit No. M-1983-052. Preliminary review of your request determined that the following I
information must be submitted to complete the requirements of Rule 4.16 of the Hard Rock/Meta] Mining Rules and
Regulations and C.R.S. 34-32-117(5) of the Colorado Mined Land Reclamation Act (a copy of this Rule and Statute aze I
enclosed for your information): I

4.16.1(2) The Operator shall include in the notice to the Office the names and addresses and phone numbers of
all owners of record to the affected land.
4.16.1(3) The written notice requesting release shall be sent by Certified Mail and be separate from other types
of communication to the Oflce.
