6.4.12 EXHIBIT L -Reclamation Costs 13. The reclamation cost estimate will need to be adequate to cover the cost of reclamation for the maximum disturbance expected including the pit excavation at its maximum size, plant site, and slurry wall installation (either 100% or 20% option). The Division will also require an itemized cost for equipment mobilization demobilization, and an indirect cost of 25% for the entire project. Please respond. 6.4.19 EXHIBIT S -Permanent Man-Made Structures 14. On page 6 of the mining plan, the applicant proposes setback distances from a variety of permanent man-made structures. However, on December 12, 2001 the applicant provided a written commitment to stay in excess of 200 feet from man made structures as part of their mining until a slope stability analysis or agreement with the owner of the structure is obtained. Just as a reminder, any stability analysis and proposed setback distances provided after the amendment decision date will need to be submitted as a modification to the permit. Please comment. Rule 1.8.1 states that changes and additions to the permit application must be filed with [he Division and the County Clerk and Recorder. The applicant must also provide the Division with an affidavit or receipt demonstrating that the change or addition was filed with the County Clerk and Recorder no later than the close of business on the day the change was filed with the Division.
