MANAGEMENT <br />RECEIVED <br />AUG 1 31998 <br />DIV. OF MINERALS <br />During the last week of July, a significant rainfall event occurred at the site. The <br />resultant run-off combined with gravel, mud and rocks plugged culvert C-9. The <br />Operator does not believe that the culvert will be recovered. Therefore, enclosed is an <br />application for a technical revision which provides for an open channel ditch to handle <br />the run-off previously handled by culvert C-9. The following changes were made to the <br />site drainage system as part of this technical revision application: <br />1) Ditch C5 will be replaced by a drop inlet (See Map 34 -Drop Inlet Detail). A <br />short new culvert C13 will be installed from the collection box to handle the run- <br />off previously collected by ditch C5. <br />2) The 4" ADS drain from the spring to Ditch C12 was added to all of the maps. <br />3) A short 36" diameter culvert C9 will replace culvert 618. <br />4) The new open channel ditch is comprised of two sections, C16 and C14. Ditch <br />C16 is a V-Ditch and C14 is a trapezoidal section. Ditch C14 will be extended <br />up to near the inlet of culvert 65. <br />5) Culvert 618, ditch 625 and culvert 65 are deleted from the drainage plan as a <br />result of this revision. <br />
