Secretary of States Office Business Center Web Page:
Transfer of Permit and Succession of Operators Application Form: Please fix Pages 2, 4 and 5.
Performance Warranty: Please fix Pages 1 and 3.
Fipancial Warranty/Certification of Deposit: Please fix Pages 1 and 4.
Assignment of Certificate of Deposit: Please change Co. to Corporation and give the bank a copy of this
Document.
Certificate of Deposit and Certificate of Deposit Copv and Certificate of Deposit Signature Card aze
being returned to you. Please take both of these forms to the bank so that they can change "DMG" to
"NLRB" which stands for Mined Land Reclamation Boazd. The CD should be assigned to the Boazd not
