Frei <br />5650 E. 88th Avenue <br />Henderson, Colorado 80640 <br />Re: WALSTRUM MINE - Permit No. M-83-033 <br />Dear Mr. Frei: <br />Action taken by the Mined Land Reclamation Board on July 25, 1985 is as <br />follows by agenda number. <br />Agenda No. 46 - The Board approved the amendment application (Revision <br />No. 2, dated May, 1985) with no change to the $41,000.00 financial <br />warranty. This amendment and all relevant correspondences becomes part <br />of the permit document. Please attach the enclosed form to the <br />original permit. <br />A~end_a _No~. 47 - The Board assessed a civil penalty of $4,300.00 based <br />on 4~days of violation at $100.00 per day. The 43 days is from the <br />date of the inspection (September 11, 1984) to the date of the Board <br />issued Notice of Violation and Cease and Desist Order (October 24, <br />1984). The Board waived $3,000.00 with the remaining $1,300.00 due and <br />payable within 90 days. (The check should be made to the State of <br />Colorado.) <br />Agenda No. 48 - The Board approved the succession of operator and <br />transfer of permit to Associated Aggregates.
