Walther <br />Highlands Ranch, CO 80126 Executive Director <br /> Michael 8. long <br /> Division Direct« <br />Re: Thomas Pit, Permit No. M-1976-020, Receipt of 112 Construction Materials Reclamation Permit <br />Amendment Application Package, AM -1/ <br />Dear Mr. Izwicld: <br />On February 7, 2002, the Division of Minerals and Geology received your 112 Construction Materials Reclamation <br />Permit Amendment Application package for the Thomas Pit, Permit No. M-1976-020. Preliminary review of the <br />information received determined that the following items must be received before the Division can consider your <br />application as being submitted and technical review can begin: <br />APPLICATION FORM: Pace 1. Item 1: The Division requites that the cEUrent permittee submit an <br />amendment application (i.e. Four Corners Materials, Inc.j. Page 7, Signature Page: The Division requires the <br />signature, notarization and acknowledgement by the corporate secretary be by the current permittce (i.e. Four <br />Corners Mahriats, lnc.). <br />In addition, the Division requires that 5 complete copies of the application package be submitted, to include 1 original <br />application and four copies and five copies of alt exhibits. <br />Your application will not be considered submitted until the information listed above is received and found sufficient to <br />begin our review. A decision date will be established 90 days from the date of receipt of all of the requested <br />information. Additionally, if you have already published notice you will need to republish notice, after the Division <br />considers the application submitted. This notice must be published once a week for four (4) conseartive weeks, starting <br />within ten (16) days of the date your application is considered submitted We will notify you when you should initiate <br />republication of your notice. The final date for receiving comments is the 20th day after the foutfh publication or the <br />next regulaz business day. <br />This letter shall not be construed to mean that there are no other technical deficiencies in your application.
