Cattany <br />Division Director <br />Natu21 Resource Trustee <br />Re: Incas Mine, Permit No. M-1986-076, Receipt of 110(d) Designated Mining Hazd Rock/Metal <br />Mining Reclamation Permit Amendment Application Package, AM - 3 r <br />Dear Mr. Bender: <br />On August 5, 2003, the Division of Minerals and Geology received your the 110(d) Designated Mining Hard <br />Rock/Metal Mining Reclamation Permit Amendment Application package for the Incas Mine, Permit No. M- <br />1986-076. Preliminary review of the information received determined that the following items must be received <br />before the Division can consider your application as being submitted and technical review can begin: <br />APPi.iCATTnN FF.F,: The Division did not receive your $875 application fee with the application <br />package. <br />F.XFIIRIT R -Site Descries Linn Qtnle 63.2(x„11: You must address Item (a) to the extent necessary to <br />demonstrate compliance with the applicable perfom~artce standard requirements of Rule 3. At a <br />minimum, you must include a description of the vegetation and soil characteristics in the area of the <br />proposed operation. The local office of the Natural Resources Conservation Service (NAGS) may <br />provide you with this information as well as recommendations for Exhibit D - Reclamation Plan; <br />F.XAiRiT R -Site Descri~ firm (Role 6_3.2(bri: You must address Item (b} to the extent necessary to <br />demonsh~ate compliance with the applicable performance standazd requirements of Rule 3. At a <br />minimum, you must identify any permanent man-made structures within two hundred (200) feet of the <br />affected area and the owner of each strcture. Permanent man-made structures include houses, barns, <br />fences, above or below ground utilities, irrigation ditches, maintained or public roads, bridges, wells, <br />pipelines, water storage impoundments, raihoad tracks, cemeteries, and communication antennas, etc. <br />Each structure should be located on Exhibit E -Map; <br />F,XATRiT R -Site DeccrlpHnn fRnie 6_4.2tc li: You must address Item (c} to the extent necessary to <br />demonstrate compliance with the applicable performance standard requirements- of Rule 3. At a <br />minimum, you must give a description of the water resources in the azea of the proposed operation. <br />Identify any streams, springs, lakes, stock water ponds, ditches, reservoirs, and aquifers which would <br />receive drainage directly from the affected area.
