General conditions, per se, are not anticipated because the work <br />is no[ being "put out to bid", but rather is being done by [he applicant. Haley & <br />Aldrich agrees, and will state in the text of the report, that not only the <br />specifications, but also the design, QA/QC, etc., cannot be changed <br />significantly. The applicant agrees to state in the Technical Revision request <br />that such chanties will not be made without prior approval by DMG. <br />(e) A quality assurance plan listing the type and frequency of testing [o assure that <br />[he till materials and [heir placement will be part of [he final design report. <br />(f) The Operator comnuts to apprising DMG of fill slope construction schedules <br />and milestones to allow inspections, as desired by DMC, to observe fill slope <br />construction a[ critical milestones. The operator further intends to document <br />such milestones as part of the geotechnical addenda to the annual reclamation <br />reports. <br />(3) Haley R Aldrich is currently performing stability analyses for the proposed slope <br />that will be included in the final design report. Computer Modeling is being <br />done using the RSS (Reinforced Slope Stability) program which is the FHWA <br />modification of the computer program XSTABL. The minimum safety factor <br />will be 1.25. <br />(4) The Operator agrees to include a "construction report" per DMG's wishes as a <br />section of the geotechnical addendum to [he annual reclamation reports during <br />years when substantial fill slope construction is affected. <br />Wildlife Issues <br />(I) No response comment necessary. <br />(2) No response comment necessary. <br />(3) The operator intends to plant a reasonable number of natives trees and shrubs on <br />the engineered fill slope, in addition to native grasses, in order not only to <br />restore wildlife habitat value, but also to mitigate visual impacts by roughly <br />mimicking surrounding terrain appearance. DOW's statement that the current <br />reclamation plan requires planting of grasses only appears to be incorrect.
