Cattany <br />Division Director <br />Delte, CO 81416 ~ C/ Natural Resource Trustee <br />1/1000 ct~ <br />Re: Beaton Dawson Gravel, Permit No. M-1983-209, Submittal of Transfer of Permit and Succession of <br />Operator Application, Performance Warranty, Financial Warranty Certification of Deposit, and <br />Certificate of Deposit <br />Dear Mr. Gilmore, <br />On August 11, 2006, the Division of Reclamation, Mining and Safety received a Transfer of Permit and Succession of <br />Operator Application, a Performance Warranty, a Financial Warranty Certification of Deposit, and Certificate of <br />Deposit, in the amount of $10,667.00, for Benton Dawson Gravel, Permit No. M-1983-209. The Division is returning <br />the Transfer of Permit and Succession of Operator Application, Performance Warranty, and Financial Warranty <br />Certification of Deposit for the following: <br />Transfer of Permit and Succession of Operator Application: <br />The Division requires the Operator name be consistent throughout this document. The Successor Operator is <br />listed as both Frank Gilmore and Benson Dawson Gravel. Benton Dawson Gravel is also referred to as both <br />"Benton Dawson Gravel" and "Benson Dawson Gravel". <br />Performance Warranty: <br />The Division requires the Operator name be consistent throughout this document. The Notarization of <br />Operator's Acknowledgement on page 3 lists Frank Gilmore as Owner/operator is Benson Dawson Gravel. If <br />Benson Dawson Gravel is the company name of the Operator, this must be noted elsewhere in this document. <br />If Frank Gilmore is the operator as an individual, Benson Dawson Gravel must be omitted from the <br />Notarization of Operator's Acknowledgement section. <br />Financial Warranty Certification of Deposit: <br />The Division requires the bank name and location be listed in the correct spaces and completed on page 2. <br />T'he Division requires the dollar amount of the Certificate of Deposit be listed alphabetically on page 2. <br />The Division requires the completion of information pertaining to the initial date and expiration date of the <br />Certificate of Deposit be listed on page 2. <br />The Division requires the Operator name be consistent throughout this document. The Notarization of <br />Operator's Acknowledgement on page 4 lists Frank Gilmore as Owner/operator is Benson Dawson Gravel.
