Identification of Interests, MR-15, Munger Mine, Permit No. C-81-020, Grand Valley Coal Co. At your request, I checked the ownership and controller information for the McClave Mine and for the Munger Mine. There were several items which need attention: The AVS system recommendation for Scott M. Kiscaden is a "conditional issue" Kentucky issued a violation to the Chloe Creek Development Corporation for failure to pay AML fees. Scott Kiscaden is listed as an officer or controller of that company. Attached to this memo are two AVS printouts that show this. 2. The AVS system recommendation for Donald Todd Kiscaden is a "deny." There is a Kentucky violation for the Black Dragon Mining Company, of which Donald Todd Kiscaden is an officer or controller. Attached to this memo are two AVS printouts that show this. John A. McNab has been removed as president and director of Grand Valley Coal Company. However, in order to update the AVS, the company needs to send official confirmation, on company letterhead, of Mr. McNab's resignation from these two offices, and the dates of the resignations. Section 2.03.4(7) of the proposed revised page A-2 of the Munger Mine permit application needs to be modified to include the ownership information for KINVEST, Inc., similar to the ownership information found in the same section on proposed revised page A-2 for the McClain Mine.
