M-1981-11 ;Receipt of 112(c) Construction Materials Reclamation Permit <br />Amendment Applicatton ac age <br />Deaz Mr. Heffner: <br />On June 21, 2007 the Division of Reclamation, Mining and Safety received your 112(c) Construction Materials <br />Reclamation Permit Amendment Application package for the Miller Pit, M-1981-112. Preliminary review of <br />the information received determined that the following items must be received before the Division can consider <br />your application as being submitted, complete, and technical review can begin: <br />APPLICATION FORM: <br />1. The Division requires the company name listed throughout the application (including within the <br />notarization) match the company name listed with the Colorado Secretary of State (as officially <br />appears "Hunt Investment Company" on the enclosed print-off from the Colorado Secretary of <br />State's website). Please correct all highlighted portions of the original application (enclosed) on <br />a new application and return it, including on page 1 where the type of organization should be <br />listed as a corporation, not LLC, and affix a corporate sea[, as required on page 8 of the <br />application. All places where the company name is mentioned must appeaz as "Hunt Investment <br />Company" exactly, including on page 3 where the ApplicandOperator and Inspection Contact <br />a e li ted. <br />2. Rule 6.2.1(2) requires that maps be signed by a registered land surveyor, professional <br />engineer, or other qualified person. The maps submitted to the Division were not signed. <br />Please resubmit a signed copy of the maps. <br />Upon correction, please return the corrected/new documents to my attention. <br />Your application will not be considered filed until the information listed above is received and found sufficient <br />to begin our review. A decision date will be established 30 days from the date of receipt of all of the requested <br />information.
