On June 18, 2007, the Division of Reclamation, Mining and Safety received a Succession of Operators Application and
Performance Warranty for the Hubbs Pit, Permit No. M-1987-088. The Division is returning the Succession of Operators
Application for the following:
The Division requires the Permittee name listed on page 5 to match the Permittee name cutrently listed on the
permit (i.e. "Ted R Hubbs Family Trust" rather than "Ted R. Hubbs Trustee").
The Division requves the notary for the permittee on page 5 to sign on behalf of a representative for the current
permittee (i.e. "Ted R Hubbs Family Trust" rather than "Ted R. Hubbs Trustee").
The Division requires the company name listed within the Notary for Successor section on page 5 match the
successor company name (i.e. "Hubbs III, LLC" rather than "Hubb III, LLC").
