<br />paragraphs (a) (i) and (a) (11) unless he delivers <br />the records to the Director or obtains written <br />appro~aal from the Director to discard the records. <br />5. Reporting of Results. The permittee shall submit <br />quarterly reports to the Director summarizing the results of the <br />monitoring required by Part II, Sections D. 1. and 2. of this <br />permit. Copies of a.ll records on injected or produced fluids, <br />and any major changers in characteristics or sources of injected <br />produced fluid, shal:i be included in the quarterly report. <br />The first quarterly report shall cover the period from the <br />date of initial injection operation through the end of that <br />quarter. Subsequent:Ly, the quarterly reports shall cover the <br />periods from January 1 through March 31, April 1 through June 30, <br />July 1 through September 30, and October 1 through December 31. <br />Each report shall be submitted to the EPA Denver Office no later <br />than thirty (30) days after the end of the previous quarter. <br />Appendix B contains 1?orm 7520-08, which may be copied and used to <br />submit the quarterly summary of monthly averages of monitoring <br />data. Daily maximum; minimum and continuous recordings shall be <br />submitted in a tabular form, developed by the permittee. <br />E. PLUGGING AND ABANDONMENT <br />1. Notice of 1?lu in and Abandonment. The permittee <br />shall notify the Director forty-five 45 days before plugging or <br />abandonment of any we>.11. <br />2. Plugging and Abandonment Plan. The permittee shall <br />plug and abandon each well as provided in the Plugging and <br />Abandonment Plan, Appendix E. This plan incorporates information <br />supplied by the permittee and modifications outlined by the EPA. <br />EPA reserves the right to change the manner in which any well <br />will be plugged if the well is modified during its permitted life <br />or if the well is not: made consistent with EPA requirements for <br />construction and mec)~anical integrity. The Director may ask the <br />permittee to update t:he estimated plugging cost periodically. <br />Such estimates shall be based upon costs which a third party <br />would incur to plug any well according to the plan. <br />3. Cessation of In'ection Activities. After a cessation <br />of operations of two (2 years, the permittee shall plug and <br />abandon any well in accordance with the Plugging and Abandonment <br />Plan, unless he: <br />(a) has provided notice to the Director; and <br />(b) has demonstrated that the well will be used for <br />monitoring purposes or has demonstrated that the <br />well will be used in the future; and <br />Page 18 of 61 <br />EPA Permit No. COS2358-0696 <br />
