<br /> <br />2. Prior to Commencing Injection. Commercial scale <br />_ operations may not commence until all corrective action is <br />complete, appropriate documentation has been submitted to the <br />EPA, the EPA has granted approval of the completed corrective <br />action, and required CBE's have been performed and submitted to <br />the EPA. <br />3. Concurrent with Commencement of Injection. The <br />permittee shall comp:Ly with the following requirements: <br />(a) The permittee shall demonstrate that each well has <br />mechanical integrity in accordance with 40 CFR <br />146.8 and paragraph (b), below. Results of each <br />test shall be submitted to the Director as soon as <br />possible, but no later than ten (10) days after <br />the demonstration; <br />(b) The permittee shall demonstrate that there is no <br />significant fliud movement into a USDW through <br />vertical channels adjacent to an injection or <br />production wellbore, by running noise logs, <br />temperature logs, or radioactive tracer surveys <br />(RATs), in accordance with procedures specified by <br />the E1?A (Appendix C). Initial, temporary <br />injection may take place, if necessary, to comply <br />with i:he procedures outlined in Appendix C; <br />(c) The permittee shall notify the EPA that <br />construction is complete and submit Well <br />Completion Report(s) (Form 7520-10) within ten <br />(10) days of completion; <br />(d) The permittee shall notify the EPA ten (10) <br />working days prior to conversion or commencement <br />of injection activities; <br />(e) The permittee shall notify the EPA ten (10) <br />working days prior to performance of any <br />tubing/casing annulus pressure tests, cement bond <br />logs {CBE's), and/or temperature surveys for the <br />purpose of demonstrating mechanical integrity; and <br />(f) The p<~rmittee shall notify the EPA ten (10) <br />working days prior to plugging any well(s). <br />4. <br />on1. <br />1 <br />(a) Method of Demonstrating Mechanical Integrity.
