On October 6, 2006, the Division of Reclamation, Mining and Safety received a Transfer of Permit and Succession of Operators
Application, a Performance Warranty, and a $125.00 Succession of Operators fee for the Surface Rock Pit, Pem3it No. M-1984-
001.The Division is returning the Transfer of Permit and Succession of Operators Application and Performance Warranty for the
following:

The Division requires Option A be initialed on page 1 when a Transfer of Permit and Succession of Operators
Application is submitted without a Financial Warranty.
The Division requlres the Successor Operator Company name match the Colorado Secretary of State's office's listing of
that company's name. You company name is listed as "Pazkerson Brothers LLC" with the Colorado Secretary of State's
office. Please list the Successor Operator company name as "Pazkerson Brothers LLC" throughout this document (please
note the word "Brothers" is not abbreviated and there is no comma listed between "Brothers" and "LLC").
Performance Warranty:
