Rule 2.03.5 Compliance Information. <br />An AVS check was accomplished on September 28, 2005, and the applicant is clear. <br />The permit is in compliance with this section with regard to (1) and (2). <br />Rule 2.03.5(3) See the rules for the text of this rule. Page 2.03-20 needs to be revised to list the <br />last two violations issued on this permit. A previous violation listed could be deleted. <br />Rule 2.03.6 Right of Entry and Operation Information. <br />The permit is in compliance with this rule. Information starts on page 2.03-22. <br />Rule 2.03.7 Relationship to Areas Designated Unsuitable for Mining. <br />The permit is, by exception, in compliance with this rule. There are no such areas. <br />Rule 2.03.8 Permit Term Information. <br />The permit is in compliance with this rule. Information starts on page 2.03-35. <br />Rule 2.03.9 Personal Injury and Property Damage Insurance Information. <br />The permit is in compliance with this rule. The initial coverage is provided at Exhibit 3. Annual <br />renewals are submitted to the Division under separate cover. They expire October 1 of each <br />succeeding year. <br />Rule 2.03.10 Identification of Other Licenses and Permits. <br />The permit is in compliance with this rule. Information starts on page 2.03-36. <br />Rule 2.03.11 Identification of Location of Public Office for Filing of Application. <br />The permit is in compliance with this rule (see page 2.03-37.1). The location is the Routt County <br />Court House for revision applications. The Permit and Permit Application Package (PAP) are <br />maintained at the mine offices. <br />Rule 2.04.3 General Requirements: Site Description and Land Use Information. <br />The permit is in compliance with this rule. Information starts on page 2.04-1. Maps 3 and 3a are <br />prepazed according to the requirements of 2.10.
