Rule 2.03.5 Compliance Information. An AVS check was accomplished on September 28, 2005, and the applicant is clear. The permit is in compliance with this section with regard to (1) and (2). Rule 2.03.5(3) See the rules for the text of this rule. Page 2.03-20 needs to be revised to list the last two violations issued on this permit. A previous violation listed could be deleted. Rule 2.03.6 Right of Entry and Operation Information. The permit is in compliance with this rule. Information starts on page 2.03-22. Rule 2.03.7 Relationship to Areas Designated Unsuitable for Mining. The permit is, by exception, in compliance with this rule. There are no such areas. Rule 2.03.8 Permit Term Information. The permit is in compliance with this rule. Information starts on page 2.03-35. Rule 2.03.9 Personal Injury and Property Damage Insurance Information. The permit is in compliance with this rule. The initial coverage is provided at Exhibit 3. Annual renewals are submitted to the Division under separate cover. They expire October 1 of each succeeding year. Rule 2.03.11 Identification of Location of Public Office for Filing of Application. The permit is in compliance with this rule (see page 2.03-37.1).
