An applicationfor all types of revisions or a permit shall <br />contain: <br />Rule 2.08.4(5)(x) An identification ojthe permit by permit nutnber or other appropriate reference <br />+vhich is the subject of the revision; <br />The application is in compliance with this rule. An application form accompanied the minor revision <br />request that identified the permit by permit number C-81-019. <br />Rule 2.08.4(5)(6) A specific description of the requested change in the terms of the permit; <br />The application is incompliance with this rule. A specific description of the requested change was <br />included, including replacement pages and drawings. <br />Rule 2.08.4(5)(c) A specific description of crny changes in the mining crud reclcrn:ation operation <br />which may have an effect on performance bond requirements of Rule 3; and <br />The application is, by exception, in compliance with this rule. No changes in the mining and <br />reclamation operation are a part of this minor revision. This minor revision is limited to submission <br />of revised ownership and control information, an updated Spill Prevention, Control and <br />Countermeasure plan, and revised text, which describes the management of water internal to the <br />mining operation (pit water). <br />Rule 2.08.4(5)(d) Snch other information as may be necessary for• the Division to determine if the <br />proposed revision will comply with the Act and these Rules. The Board or• Division may reasonably <br />reques7 additional information to evahrcrte the proposed revision. <br />
