Release Request Documentation <br />Coal Creek Sand Resource <br />Permit M-1988-044 <br />Following is the required information for requesting a full release of Perfotn~ance and Financial <br />warranties on a portion of the Coal Creek Sand Resource operation. This request has been <br />prepared in accordance with Rule 4.17 of the Mineral Rules and Regulations of The Colorado <br />Mined Land Reclamation Boazd for the Extraction of Construction Materials. The version of the <br />rules used in preparing this request is the amended rules for May 2004. <br />Rule 4.16.1 -Operator Requirements <br />(1) A written request for release: The required letter is included. <br />(2) Land owner names, addresses, and phone numbers: <br />Colorado State Board of Land Commissioners <br />Room 620 <br />1313 Sherntau Street <br />Denver, CO 80203 <br />(303) 866-3454 - contact Mark Davis <br />(3) Notice has been sent by Certifted MaiG <br />(4) Signed Statement: see item (1). <br />This request pertains solely to the 480 acre parcel in Sections 11 and 14. All affected land within <br />this parcel was previously released in 2002. Afrer release, no permitted land will remain in <br />Sections 11 and 14.
