We have attached a copy of the eceipt we <br />retained at time of mailing. In an attempt to determine the final <br />disposition of the notice we have submitted forms to the U.S. <br />Postal Service to track the certified letter. <br />The U.S. Postal Service stated that the form would fir t have to <br />be forwarded to San Francisco, then to Oahu, and th to that <br />
