Long <br />Division Director <br />On February 2Q 2002, the Division of Minerals and Geology received an Application for Succession of <br />Operator, Performance Warranty and a $125.00 fee. And on October 3, 2001, the Division received a <br />rider to the Liberty Mutual Insurance Company Corporate Surety Bond No. 14-006-861, for the Piceance <br />Pit, Permit No. M-2001-077. The Division is returning the Application for Succession of Operator and <br />the rider for the following: <br />The Division requires that the current permittee name remain the same throughout this document, <br />including the notary (i,e.
