Please correct. <br />C) 303 spring series is not shown on the map. Please correct. <br />D) 2005/8 and site 8 are shown at two different locations with the same elevation. <br />Please determine which is correct modify map accordingly. <br />E) 115 is shown twice on the map. Please correct as appropriate. <br />F) The location of well 93-AMV-1 still remains a mystery on Map 13A. Also, AVM- <br />2 is depicted twice. Which is correct? Is the annotation AVM-2 supposed to <br />represent 93-AVM-2? If so, please use consistent labeling convention between Table <br />D and Map 13A. Please correct all information as appropriate. <br />3. The appropriate subsidence information should also be presented under TR-24 for <br />each structure to be undermined in the state coal lease. A subset or figure of the <br />appropriate part of Map 23B would be acceptable. Also, the appropriate text <br />detailing the worst possible consequences, the subsidence control plan, and <br />subsidence monitoring is required. At a minimum, CR #33, telephone cable, Foidel <br />Creek, and a small powerline appear to be within the proposed affected area. <br />4. The bond pages for TR-24 were reviewed by the Division staff and the following <br />section details the results of this review. Please correct pages as necessary and re- <br />submit. <br />1. Page 2.05-54(f)(59a) -The section on earthwork production with a DSH dozer <br />should be modified to show varying production for each task (mud pits 167.32 <br />LCY/hr, ripping 1238.76 BCY/hr, grading 121.54 LCY/hr, topsoil <br />replacement 46.11 LCY/hr), since the correction factors (push distance, <br />material weight, grade, etc.) vary for each task. <br />The following are all on Page 2.05-54(f)(596). <br />2. Mid pits should be <br />8,808 LCY/167.32 I.CY/hr = 52.63 hrs @ $62.60/hr = $3294.79. <br />3. Ripping should be <br />2.75 ac @ 2' deep = 8,873 BCY/1238.76 BCY/hr = 7.16 hrs @ $65.88/hr <br />_ $471.89. <br />
