7. Rule 2.05.3(2)(b) requires a detailed narrative and appropriate map(s) sufficient to describe
all surface disturbances to be associated with the underground mining operation. The
currently approved permit does not include descriptions of each of these surface
disturbances. Discussion should be included for each disturbed area and include
information on when this disturbance was created, specifically where it is located, what it
is presently used for, and reference to the reclamation plan details as appropriate. The
dismrbances in Apache Canyon, water tanks, refuse area and roads, as examples, should
be included.
8. Rule 2.05.3(3)(a) requires a narrative description of each mine facility. Presently, the
currently approved permit presents a table of facilities and very minimal discussion. Each
building, structure, utility corridor, should be described and referenced by the same name
as is shown on the map. Structures should be clearly identified as existing at the time of the
original permit issuance and those which were constructed after the permit was issued.
9. Rule 2.05.3(3)(c) requires descriptions, of roads, conveyors, and rail systems. Presently,
this section of the permit describes only roads. Please add the appropriate information for
the conveyors and rail system at this site.
10. Text under Rule 2.05.3(4) indicates the area north of ditches D11 and D19 was topsoiled,
mulched, and seeded in September of 1990, but Map 11 and the 1995 Annual Reclamation
Report map indicate 1991. Correct this information as appropriate.
11. On page 2.05-9, Map 133 is referenced. This is likely a typo and should read Map 13.
Please correct as necessary.
12. Page 2.05-10a (revised 10/30/89) should be re-submitted to be a one-sided page.
13. Figures 3 and 4 should have page numbers 2.05-12 and 2.05-13, respectively.
14. Page 2.05-16, Table 23 should be submitted as a one-sided page.
15. Pages 2.05-27 to 2.053 are either improperly numbered or missing numbers. Correct this
information as necessary.
16. Page 2.05-43 should be a one-sided page.
17. Page 2.05-33 states Figure 6 depictsthe final configuration of the RDA pile, with an access
road running up the face. This contradicts later information and on-the-ground
configuration. Please correct and remove information from this section as appropriate.
-8-
