Based on the data attached, the
following conditions should be placed to modify commitments 24, 30, and 31.
Condition No. 1
a. If the parameters described for Arequa Gulch underdrain in the table titled "30 Day
Average parameters and Standards that Trigger an Emergency Response by the Permittee"
exceedecJ, the ermittLe~epCmtUts~tpcontclvud hat a t"t(((~a~(11,u ~¢yQrp m Invent (allure" (~ oRQA~tD~e~nm-T
I +M1r RGTtsld GArI clu 1 ~OC~U~T ~~g Tj~C~TlonlS ON10UNP 'i .,~
s) has or s a ou to or emer a response Regg.
purposes. The 30 day average for the underdrain is greater than .5 WAD cyanide - 8.5
pH (no volume standard). If the 30 day average for the LDS is greater than 5 WAD
cyanide 8.5 pH, and less than 28 gallons/day.
b. If cyanide concentrations at any of the ground water monitoring points in Arequa or
Wilson Gulches exceed the parameters described in the table titled "30 Day Average
Parameters and Standards that Trigger an Emergency Response by the Permittee", then the
permittee must conclude that a "failure or imminent failure" (loss of containment of
cyanide solution) has or is about to occur for emergency response purposes. For the GW
monitoring wells, the WAD cyanide is greater than .2 mg/I and greater than 8.5 pH (no
volume standard).
c. If either (a) or (b) occur, the permittee shall notify the Office according to the provisions
of Rule 8.1, and 8.2. In addition to the requirements of Rule 8.2.2, the notice shall
include copies of the actual laboratory analyses report data sheets, the date samples were
collected, the sample points sampled, the method of sample preservation, and the name
of the person that collected the samples.
