Section 2.10 to include reference to an exhibit of this section where current proofs of <br />publications (the most current proof being for Permit Renewal 3) may be found. <br />Item 5. Rule 2.04.3(2). Section 3.1.4 of the PAP on page 3-1 (03-1-95) appears to supersede <br />information on pages 3-7, 3-8, and down to section 3.2 on page 3-9 (all annotated <br />ABC/1145/900301). (The Division copy of the PAP has a note on these pages to retain them in <br />the PAP.) If the information is superseded, and to be removed, a blank page 3-8 (annotated "This <br />page intentionally left blank.") and a revised page 3-9 that deletes the words "of these existing <br />land use zones." would be appropriate. If some of this information is to remain in the PAP, <br />reformatted pages [o systematically present the information are needed. <br />Item 6. The Division copies of the PAP contain two page 4-iv. Both contain current, but not <br />identical, information. Page 4-iv (ABC/1145/9105090), the List of Tables, might be added as <br />page 4-vii. <br />Item 7. Section 4.2.3 (PAP) puts Sections 4.1 through 4.2.2 in perspective. However, there are <br />discrepancies. The following requests and suggestions are made to more clearly demonstrate <br />compliance with the requirement of Rule 2.03.3(1). <br />Item 7a. Please reconcile the schedule of reclamation on pages 2-18 (Section 2.6.2, May 84 -Sep <br />94), page 4-9 (Section 4.2.3, May 93 -Sep 93), and page 4-54 (Section 4.4.1, May 904 -Aug <br />94). This section might be changed to read "The reclamation effort commenced in (reconciled <br />date), and demolition, backfilling, grading and seeding was completed by (reconciled date)." <br />Although nearly all of the reclamation work was completed, there is some work remaining and <br />the reclamation phase will continue until final bond release. <br />Item 7b. Information in Section 4.2.4.1 (page 4-10) does not demonstrate that the road is <br />crowned to prevent ponding [Rule 4.03.1(4)]. Please add to this section a statement that the roads <br />are surfaced with anon-acid, non-toxic-forming gravel, and crowned to prevent ponding. <br />Item 7c. PAP, Page 4-11. Please revise this section to recognize that County Road 335 no longer <br />crosses the permit boundary for the 2" water line. <br />Item 7d. Suggestion. All the remaining roads might be re-designated via a Technical Revision as <br />light-use roads, whereby the criteria for permanent retention are significantly relaxed. { In this <br />regard, certifications for the haul were found in the PAP, certifications for the permanent access <br />roads were not.) Culverts to remain permanently need to be identified. <br />Item 8. Section 4.4.1. The date of August 1994 on PAP page 4-54 needs to be reconciled with <br />September 1994 on PAP page 2-18. <br />Item 9. Blank spacer pages 4-64 and 4-65 are required for pagination of the PAP. <br />Item 10. Section 6 of the PAP has proven to be a valuable aid to administration of the permit; <br />however, it is outdated. Please consider updating this section. <br />
