Asbestos Containing
Materials Survey
4629 Nodh Ovedand Trail
La Porte, Colorado
1. Introduction
ARCADIS G&M, Inc. (ARCADIS) has compieted an ACM survey at the Holcim, Inc.
Plant located at 4629 North Overland Trail, La Porte, Colorado. ARCADIS utilized
Landmark Environmental, Inc. (Landmark) to complete the sampling and reporting
portions of the scope of work.
The purpose of the ACM survey was to identify the presence, location, quantity,
~ri~bility, and condition of ACMs located within the structure. The survey was
cotducted according to EPA Asbestos Hazard and Emergency Response Act
(AHERA) and Occupational Safety and Health Association (OSHA) 29CFR1926.1101
protocols, as well as generally accepted industry standazds. Asbestos inspection
activities were conducted by AHERA and CDPHE accredited personnel.
2 Asbestos Sample Collectlon and Analysis
Bulk samples of the suspect ACMs were collected in a random and representative
manner, as determined by the inspector. Inspectors collected bulk material samples by
taking a composite core that includes all layers within the suspect ACM.
The bulk suspect asbestos samples were submitted to Reservoirs Environmental
Services, Inc., (Reservoirs) located in Denver, Colorado. Reservoirs is a National
Voluntary Laboratory Accreditation Program (NVLAP) accredited laboratory and is
accredited by the American Industrial Hygiene Association (AIHA). The samples
were analyzed by Polarized Light Microscopy (PLM) to determine asbestos type and
content. If PLM analysis identified less than 3 percent asbestos (except in window
glazing putty samples), additional point count analysis was performed to more
accurately determine asbestos content in those materials.
The total asbestos reported is the average of all components in the material, with the
asbestos content of separate layers also identified. Unused portions of samples are
azchived for 60 days unless the client requests special handling. Laboratory analysis
sheets are included in Appendix C.
t~ nots~woi a,n..m"9mia ~wecsvbanwvNmmmwwy~+~wnn ~oul+~aotdoc
