Verification procedures are included in relevant sections of the
analytical laboratory SOP in Appendix A.
5.5 Reoortine
The analytical laboratory will produce a report which Comprises:
(1) A statement of the laboratory test results, and
' (2) A quality assurance report.
The report will be prefaced by a sheet which identifies the
analytical laboratory, the laboratory job number, the client and
the date of the report. Certification is provided by th@ inclusion
of this page which will be signed and dated by an authorized
' representative of the analytical laboratory together with their
title.
' 5.5.1 Analytical Results Report: The analytical test results will be given
on laboratory data sheets which contain the following ir{formation:
' (1) The date of the report and the laboratory performing the
analysis;
' (2) The client identification number, the name of the client
and the laboratory identification number;
'
(3) The date and time the samples were collected, the date
' and time the samples were received, the date of analysis
and the technician that performed the analysis; and
' (4) A description of the test performed, the final result,
the detection limits and dilution us@d (where
