Mr. Brown if you sign the application on behalf of Yuma County <br />then your signature is the one the notary needs to notarize, and she needs to <br />affix her seal on the page. <br />implication Fee: The Division did not receive the 112 amendment application fee <br />of $1,550.00. <br />Performance Warranty: The Division did not receive the original properly executed <br />performance warranty. <br />EXHIBIT N - Other Permits and Licenses: You must submit a statement identifying <br />any other permits, licenses and approvals that you hold or will be seeking in <br />order to conduct mining and reclamation operations on the amendment permit. <br />You may amend your application by submitting the requested items. In accordance with <br />Rule 1.5, a new decision date will be established 90 days from the date of receipt of <br />all requested items. If you have already initiated publication, you will need to <br />republish notice which specifies the final date for receiving comments is the 20th day <br />after the fourth publication or the next regular business day. <br />This letter shall not be construed to mean that there are no other technical <br />deficiencies in your application.
