T. ~ R. 98W 6TH Meridian <br />County: Rio Blanco State: Colorado <br />WELL/FACILITY NUMBER: 1 P.-SHR OPERATOR: White River Nahcolite <br />SURFACE OWNERSHIP: Federal Indian Fee State <br />LEASE NUMBER: C-0118327 UNIT NAME/CA NUMBER: None <br />TYPE OF EVENT: Blow out Fire Fatality Injury Property Damage <br />Oil Spill Saltwater Spill Toxic Fluid Spill Gas Venting <br />OiUSaltwater Spill Oil/Toxic Fluid Spill <br />Other (specify): Barren liquor (Production water injected into solution mine) <br />CAUSE OF EVENT: Durin¢ a Rant outage an attempt to send wash water to production wells_ was made to <br />prevent scaling during start-up. A valve was opened at well 1 A-SHR during the attempt to supply wash water <br />Later. a different crew started the r~lant up and did not realize a wash water valve was ooen at 1 A-SHR. Also. <br />a valve and cross over hose from wash water to barren aipelines (mid wav in the well field! was ooen which <br />contributed to the cause <br />VOLUMES OF POLLUTANTS: Dischazged/consumed: Approximately 20.000 gallons <br />Recovered: 18.060 uallons. Therefore -2.000 was discharged from <br />containment structure. <br />TIME REQUIRED TO CONTROI. EVENT (in hours) Immediately turned off. but undetected for 2-8 hours. <br />ACTION TAKEN TO CONTROL EVENT, DESCRIPTION OF DAMAGE, CLEAN UP PROCEDURES <br />AND DATES: As a result of dailr• well Seld inspections leak was stooped eazly 2-14 . The valve was locked <br />and tagged out until procedures anti check list is developed for start-up and shut-down of the wash water <br />system. After contacting a su~rvisor and EHS manager. a water truck was utilized to remove all standing <br />waterer 1430 on 2/14/99. After area dries up. if bicarb deposition is visible. surface material will be removed <br />for disposal. An action plan to mitigate hi¢h TDS water and determine damage (submitted I-20-99) will be <br />basis for corrective action. Approximately one foot of snow and winterls~ng_precipitation will dilute spill. <br />CAUSE AND EXTENT OF PERSONNEL INJ[JRY: None <br />OTHER FEDERAL, STATE AND LOCAL GOVERNMENT AGENCIES NOTIFIED: DMG and WOCD <br />ACTION TAKEN TO PREVENT :RECURRENCE: Valve at lA-SHR will remain locked until procedures for <br />operation (start-up and shut-downZ of the wash water system are developed and approved by the BLM and <br />DMG. The storage capacity of muc~it at 1 A-SHR will be restored.
