Preliminary review of the information received determined that the <br />following information must be received before the Division can consider the <br />application ae being submitted and technical review can begin: <br />APPLICATION FEE: The Division did not receive the $1,500.00 conversion <br />fee. <br />EXHIBIT F - Reclamation Plan Man (Rule 6.4.6): <br />The map(s) of the proposed affected land, by all phases of the total scope <br />of the mining operation, shall indicate the following: <br />(a) The expected physical appearance of the area of the affected land, <br />correlated to the proposed mining and reclamation timetables. The map <br />must show proposed topography of the area with contour lines of sufficient <br />detail to portray the direction and rate of slope of all reclaimed lands; <br />and <br />(b) Portrayal of the proposed final land use for each portion of the <br />affected lands. <br />anninia v rruvi or nailino of noc>_ces to t=ounty wmm>_e s>_oners <br />(Rule 6.4.141 You must submit proof of delivery of the Notice of the <br />Application form to the Rio Grande County Commissioners. <br />xu3.e i.o.z ul(D1 - nttioavit ox Foetina notice at the Location ox Mine <br />Site: You must poet notices at the location of the proposed mine site, of <br />sufficient size and number to clearly identify the Bite as the location of <br />a proposed mining operation giving your name, address, and phone number, <br />and stating that you have applied for a mining permit with the Colorado <br />Mined Land Reclamation Board. Your notice must also state that anyone <br />wishing to comment on the application may view the application at the <br />Montezuma County Clerk and Recorder's Office and should send comments <br />prior to the end of the public comment period to the Colorado Mined Land <br />Reclamation Division, Room 215, 1313 Sherman St, Denver, Colorado 80203. <br />
