Conversion Application Package
Dear Ms. Benton:
On October 6, 1995, the Division of Minerals and Geology received the 110(2) to
112 Reclamation Permit Conversion Application package for the 374 Pit, Permit
No. M-87-020. Preliminary review of the information received determined that the
following information must be received before the Division can consider the
application ae being submitted and technical review can begin:
APPLICATION FEE: The Division did not receive the $1,500.00 conversion
fee.
EXHIBIT F - Reclamation Plan Man (Rule 6.4.6):
The map(s) of the proposed affected land, by all phases of the total scope
of the mining operation, shall indicate the following:
(a) The expected physical appearance of the area of the affected land,
correlated to the proposed mining and reclamation timetables. The map
must show proposed topography of the area with contour lines of sufficient
detail to portray the direction and rate of slope of all reclaimed lands;
and
(b) Portrayal of the proposed final land use for each portion of the
affected lands.

Application form to the Rio Grande County Commissioners.

Site: You must poet notices at the location of the proposed mine site, of
sufficient size and number to clearly identify the Bite as the location of
a proposed mining operation giving your name, address, and phone number,
and stating that you have applied for a mining permit with the Colorado
Mined Land Reclamation Board. Your notice must also state that anyone
wishing to comment on the application may view the application at the
Montezuma County Clerk and Recorder's Office and should send comments
prior to the end of the public comment period to the Colorado Mined Land
Reclamation Division, Room 215, 1313 Sherman St, Denver, Colorado 80203.
