Long <br />p rvuinn Dueuor <br />On November 3, 1995, the Division of Minerals and Geology received the <br />112 Rec:Lamation Permit Amendment Application package for the above referenced <br />operation. Preliminary review of the information received determined that the <br />following information mu et be received before the Division can consider the <br />application as being submitted and technical review can begin: <br />Rule 1.6.2(1)(b) - Affidavit of Posting Notice at the Location of Mine Site: <br />You must post notices at the location of the mine site, of sufficient size <br />a~td number to clearly identify the site as the location of a mining operation <br />that is being amended, giving your name, address, and phone number, and <br />stating that you have applied for an amendment to a mining permit with the <br />Colorado Mined Land Reclamation Board. Your notice must also state that <br />anyone wishing to comment on the application may view the application at the <br />Montezuma County Clerk and Recorder's Office and should send comments prior <br />tq the end of the public comment period to the Colorado Mined Land <br />Rs~c lamation Division, Room 215, 1313 Sherman St, Denver, Colorado 80203. <br />After posting this notice you must provide a signed affidavit that such <br />notice has been posted according to the provisions of this Aule and submit <br />the affidavit to the Division. <br />You may amend the application by submitting the information listed above. In <br />accordance with Rule 1.8, a new decision date will be established 90 days from the <br />date of receipt of all of the requested information. Add itionaLly, if you have <br />already published notice you will need to republish notice, after the Divie ion <br />consideres the application submitted, once a week for four (4) consecutive weeks, <br />starting within ten (10) days of the date the application is considered submitted. <br />The final date for receiving comments is the 20th day after the fourth publication <br />or the next regular business day. <br />This letter shall not be construed to mean that there are no other technical <br />deficien~~ies in your application.
