SHELTON, Dlrsctor ~=-~~'`~~°• ~~~~_~~~ ~~ G' <br />f <br />LIMITED IMPACT (110) AND <br />SPECIAL TEN-DAY (111) PERMIT <br />APPLICATION FORM <br />Application for Mining and Reclamation Permit, pursuant to C.R.S. 1973, $4-32-101, <br />et sec, as amended. <br />The following explanations cover Limited Impact applications (110's) and Special <br />(Ten-Day Processing - 111's) applications. In the interest of efficient processing, <br />please check the appropriate box to indicate which type of application y u are applying <br />for. 111 permits may be issued only under special circumstances. Be su a you are <br />eligible. <br />~ LIMITED IMPACT OPERATIONS must disturb less than ten acres (in luding all <br />roads, stockpile areas and any other surface facilities) over he life of the <br />mine, and must extract LESS THAN 70,000 tons of mineral, overb rden or <br />combination thereof per calendar year. If the affected area i some fraction <br />greater than nine acres, be sure NOT to round this figure up t ten acres in <br />the narratives or elsewhere. <br />I_I SPECIAL (ten-day processing) operations are any sand, gravel, r quarry <br />aggregates operations which are to be operated for the sole pu pose of <br />obtaining materials for highway, road, utility, or similar-typ construction <br />under a Federal, State, county, city, town, or special distric contract where <br />the contract calls for work to be commenced within a specifically short time <br />and which will affect ten acres or less (including all roads, tockpile areas, <br />and any other surface facilities). <br />INSTRUCTION: This application and all accompanying materials as outline below must be <br />submitted in duplicate.
