1 <br />' full, the laboratory will so note on the chain-of-custody <br />form); <br />(3) that the labels affixed to the sample containers are <br />' legible and are in agreement with both the chain of <br />custody and the request-for-analysis form. <br />Having met all these requirements the laboratory will indicate on <br />the chain of custody form that the samples were received in a <br />"good" condition. Any broken sample containers, or noncompliance <br />with the above items, will be detailed on the chain of custody. <br />The laboratory will then check the pH of all preserved samples to <br />assure that the sample has been properly preserved. <br />' 2.1.12 Laboratory Quality Control Check: Any nonconformity with the quality <br />control guidelines must be noted on the chain of custodly, and the <br />' laboratory is required to contact the field sampling personnel by <br />telephone, to inform him of such conditions. <br /> <br /> <br /> <br /> <br />1 <br /> <br /> <br /> <br />All such deviations from procedure will be noted and transmitted to <br />the Project Manaqer. If no significant deviations from the <br />procedure are encountered the samples will proceed to analysis. If <br />significant deviations are encountered, additional samples will <br />need to be collected according to these protocols and resubmitted <br />to the laboratory for analysis. <br />The quality of the laboratory analytical results will be <br />periodically checked by submitting prepared spike samples (with a <br />documented cyanide concentration known to BMR and not to the <br />laboratory). No more than four of these spiked samples will be <br />submitted in the first year. Thereafter, the frecjuency of <br />additional spike samples will be evaluated based on iChe lab's <br />performance. <br />-10- <br />1 <br />
