Re: Florence Sand & Gravel Pit, Permit No. M-1992-051, Application for Succession of Operators, SO-Ol
Dear Mr. Rodarmel:
On August 14, 2006, the Division of Reclamation, Mining and Safety received a revised copy of an Application for
Succession of Operators, a $125 fee, an amendment to Fremont Bank Letter of Credit No. 124, and a Performance
Warranty for the Florence Sand & Gravel Pit, Permit No. M-1992-051. The Division is retluning the Succession of
Operators Application, Letter of Credit Amendment, and Performance Warranty for the following:
Succession of Operators Applicafion:
The Division requires the Current Permitted Operator Company name on page 2 match the operator name
currently listed on the permit. The operator name cturently listed on the permit is Florence Sand & Gravel.
The Division requires the Current Operator name (i.e. Florence Sand & Gravel) and the Successor Operator name
(i.e. Florence Sand and Gravel LLC) to be consistent throughout this document.
T'he Division requires that the name on Successor Operator name be consistent with the company name registered
with the Secretary of State's office (i.e. Florence Sand and Gravel LLC).
The Division requires the Notary for Permittee and Notary for Successor sections on page 5 be signed For the
permittee and successor operator, not for the notary.
Performance Warranty:
The Division requires the Operator listed throughout this document match the Successor Operator Company name
listed on page 2 of the Succession of Operators Application.
The Division required the Operator name be consistent throughout this document (i.e. Florence Sand and Gravel LLC).
The Division requires the completion of the second "Operation" blank on page 1.
The Division requires the Notarization of Operator's Acknowledgement on page 3 be signed for the operator, not
for the notary.
Letter of Credit Amendment:
The Division requires that the name on company name listed on this document be consistent with the company
name registered with the Secretary of State's office (i.e. Florence Sand and Gravel LLC}.
Upon correction, please return these documents to my attention.
