Financial Warranty Corporate Surety and Performance Warranty
Dear Mr. Onorofskie:
On July 27, 2007, the Division of Reclamation, Mining and Safety received a revised Succession of Operators
Application, a supplemental $19.00 to complete the application fee, Safeco Insurance Company of America Corporate
Surety Bond No. 6503920, in the amount of $1,429,700.00, and a revised Performance Warranty for the Eagle-Gypsum
Mine, Permit No. M-1984-041. The Division is returning the Financial Warranty Corporate Surety and Performance
Warranty for the following:
Financial Warranty Corporate Surety:
The Division requires the company name listed within the Notazization of Operator's Acknowledgement on page
6 match the operator's company name (i.e. "American Gypsum Company LLC" rather than"American Gypsum
Co. LLC"). Please have your notary make and initial this correction.
Performance Warranty:
The Division requires the company name listed within the Notarization of Operator's Acknowledgement on page
3 match the operator's company name (i.e. "American Gypsum Company LLC" rather than"American Gypsum
Co. LLC"). Please have your notary make and initial this correction.
Please also fmd the inadequate application form and Performance Warranty that you submitted on July 27, 2007 enclosed
for your records.
Upon correction of the revised Financial Warranty Corporate Surety and Performance Warranty, please return these
