On April 25, 2002, the Division of Minerals and Geology received your the 110 Construction Materials
Reclamation Permit Amendment Application package for the Coaldale Gravel Pit, Permit No. M-1978-096.
Preliminary review of the information received determined that the following items must be received before the
Division can consider your application as being submitted and technical review can begin:
APPLICATION FEE: The fee fora 110 Constructian Materials Reclamation permit Amendmenr is
$719.00. You have provided a check for $875.00. Your check No. 061686 is being returned to you.
Please provide the correct apphcation fee of $719.00 in acceptable form.
APPLICATION FORM: Only the original application was provided. Please follow the requiremetrts
of Rule 1.10(2) and provide two copies of the application form and its exhibits.
Paces 4 and 5 of the aoolicafion form: The initials provided on Pages 4 and 5 are "PG". The
individual signing on behalf of Fremont Cou~y is James R. Schauer. Please provide revised versions
of Pages 4 and 5, which Mr. Schauer has initialed.
